Beginning April 21, 2025, patients can be referred for in-home physical therapy appointments by selecting “Ambulatory Referral to Physical Therapy – In-Home Visits” in Epic. This order will be routed to Luna, a platform that matches patients with licensed physical therapists based on specialty, location, and availability. To ensure continuity of care, patients will work with the same therapist throughout their outpatient treatment plan.
